Post-COVID Rehabilitation: Causes, Symptoms and How Physiotherapy Can Help

Learn about post-covid syndrome, its causes, symptoms, and how physiotherapy in Malaysia can help you recover effectively without surgery.

Months after recovering from COVID-19, many Malaysians find themselves still battling breathlessness, fatigue, brain fog, and persistent aches. Post-COVID syndrome – sometimes called "long COVID" – affects an estimated 10-20% of people who contract SARS-CoV-2, regardless of whether their initial illness was mild or severe. As Malaysia moves beyond the pandemic, the lasting health impact on the population demands effective rehabilitation, and physiotherapy is at the forefront of this recovery effort.

What Is Post-COVID Syndrome?

Post-COVID syndrome is defined as symptoms that persist or develop more than 4 weeks after the initial COVID-19 infection that cannot be explained by an alternative diagnosis. The World Health Organization defines the condition as occurring within 3 months of the acute infection, with symptoms lasting at least 2 months. It is a multi-system condition that can affect the lungs, heart, brain, muscles, joints, and psychological well-being simultaneously.

The condition does not discriminate – it affects people who were hospitalised on ventilators and those who had seemingly mild infections managed at home. Young, previously healthy adults are affected alongside older Malaysians with pre-existing conditions.

Why Symptoms Persist

Research into post-COVID syndrome is ongoing, but several mechanisms have been identified:

  • Persistent inflammation – Residual inflammatory processes continue to affect multiple organ systems even after the virus has cleared
  • Deconditioning – Prolonged bed rest and inactivity during illness lead to significant loss of muscle mass, cardiovascular fitness, and functional capacity
  • Autonomic nervous system dysfunction – Disruption of the autonomic nervous system can cause heart rate abnormalities, blood pressure changes, and exercise intolerance (dysautonomia)
  • Microclotting and vascular damage – Tiny blood clots and damage to blood vessel linings may impair oxygen delivery to tissues
  • Neurological involvement – The virus can affect the nervous system, contributing to brain fog, fatigue, headaches, and altered sensation
  • Respiratory damage – Lung scarring (fibrosis) and reduced lung capacity in those with severe initial infections

The Many Faces of Post-COVID Symptoms

Post-COVID syndrome presents with a wide range of symptoms that can fluctuate in severity:

  • Fatigue – The most common and often most debilitating symptom. A profound, disproportionate exhaustion that is not relieved by rest and can worsen dramatically after physical or mental exertion (post-exertional malaise)
  • Breathlessness – Difficulty breathing during activities that were previously easy, such as climbing stairs, walking briskly, or even talking at length
  • Brain fog – Difficulty concentrating, word-finding problems, poor memory, and slowed mental processing that can significantly impair work and daily function
  • Muscle and joint pain – Widespread aching, weakness, and reduced exercise tolerance
  • Chest pain and palpitations – Ongoing chest tightness and awareness of rapid or irregular heartbeat
  • Sleep disturbance – Insomnia, unrefreshing sleep, and disrupted sleep patterns
  • Anxiety and depression – Both as direct neurological effects and as psychological responses to prolonged illness

If you are experiencing persistent symptoms after COVID-19, it is important to seek professional assessment for a thorough evaluation.

The Physiotherapy Rehabilitation Programme

Post-COVID rehabilitation requires a careful, individualised approach. Unlike standard exercise programmes, recovery must account for the unpredictable nature of symptom fluctuation and the risk of post-exertional malaise.

Thorough Assessment

Your physiotherapist will assess your breathing pattern, exercise capacity (using tests such as the 1-Minute Sit to Stand), oxygen saturation levels, functional ability, and screen for any red flags requiring medical referral. This baseline assessment guides your personalised programme.

Breathing Rehabilitation

Restoring normal breathing patterns is often the first priority:

  • Diaphragmatic breathing retraining – Many post-COVID patients develop dysfunctional breathing patterns, relying on upper chest and accessory muscles. Restoring diaphragmatic breathing improves oxygenation and reduces breathlessness
  • Breathing control techniques – Pursed lip breathing and paced breathing to manage breathlessness during daily activities
  • Airway clearance techniques – If persistent cough or sputum production is present, specific techniques help clear the lungs effectively
  • Thoracic mobility exercises – Gentle stretches and movements to reduce chest wall stiffness and improve lung expansion

Graduated Return to Exercise

The approach to exercise must be cautious and paced to avoid triggering post-exertional malaise:

  • Phase 1: Gentle movement – Flexibility exercises, breathing-focused movement, and very light walking. Monitor symptoms for 24-48 hours after each session
  • Phase 2: Low-intensity activity – Gentle walking, light stretching, and basic strengthening exercises. Heart rate kept below an individually determined threshold
  • Phase 3: Moderate activity – Progressive strengthening, increased walking duration, and introduction of activities like cycling or swimming
  • Phase 4: Return to baseline – Gradual return to pre-COVID activity levels, sport, and exercise routines

Pacing and Energy Management

Learning to pace activities is essential for managing fatigue. Your physiotherapist will help you understand your "energy envelope" – the amount of activity you can tolerate without triggering a symptom crash. Strategies include breaking tasks into smaller chunks, prioritising important activities, planning rest periods, and using the "stop before you need to" principle.

Malaysian-Specific Rehabilitation Considerations

  • Exercise timing matters – in Malaysia's hot, humid climate, exercising during cooler morning or evening hours is important, as heat and humidity increase cardiovascular strain
  • Stay well hydrated – dehydration worsens fatigue, brain fog, and dizziness. In Malaysia's climate, fluid intake needs are higher
  • Aquatic therapy is valuable – warm water pools provide buoyancy support, making exercise more comfortable for deconditioned patients. Many physiotherapy centres in Malaysian cities offer hydrotherapy
  • Nutritional support aids recovery – ensure adequate protein intake (at least 1.2g per kg of body weight) to support muscle rebuilding, and maintain vitamin D levels despite limited sun exposure in air-conditioned indoor lifestyles
  • Community support groups – connecting with other post-COVID patients through online Malaysian support communities can provide valuable emotional support and shared strategies

Reclaim Your Life After COVID

Post-COVID syndrome is a real and recognised condition, and recovery is possible with the right approach. Do not accept persistent fatigue, breathlessness, or brain fog as your new normal.

A qualified physiotherapist experienced in post-COVID rehabilitation can assess your specific situation and create a treatment plan designed for your needs. Find a physiotherapist near you and take the first step toward getting back to the activities you love.
