What to expect from physiotherapy rehabilitation after total knee replacement surgery in Malaysia, including recovery timeline and exercises.

Total knee replacement (TKR) is one of the most common orthopaedic surgeries performed in Malaysia, particularly for patients with severe osteoarthritis. Physiotherapy is absolutely essential for full recovery after this procedure.

Why Physiotherapy is Essential

Without proper rehabilitation, a knee replacement can result in poor range of motion, persistent stiffness, and reduced function – even though the surgery itself was technically successful. Physiotherapy ensures you regain the full benefit of your new joint.

Recovery Timeline

Week 1–2: Hospital Phase

Physiotherapy begins within 24 hours of surgery, usually while you are still in hospital. The focus is on gentle knee bending and straightening, getting out of bed safely, walking with a walker or crutches, and managing swelling with ice and elevation.

Week 2–6: Early Recovery

After discharge, you will need regular physiotherapy sessions (2–3 times per week). Goals include achieving 90 degrees of knee flexion, walking with reduced dependence on walking aids, climbing stairs with support, and performing basic daily activities independently.

Week 6–12: Active Rehabilitation

This is the most intensive phase. Your physiotherapist will progress you through strengthening exercises, balance training, and functional activities. By week 12, most patients achieve 110–120 degrees of knee flexion and can walk confidently without aids.

Month 3–6: Return to Full Activity

Continued strengthening and functional training. You can gradually return to recreational activities like swimming, cycling, and light hiking. Full recovery typically takes 6–12 months.

Key Exercises After Knee Replacement

  • Quad Sets: Tighten the thigh muscle with the leg straight. Hold 5 seconds, repeat 10 times.
  • Heel Slides: Lying down, slide your heel toward your buttock by bending the knee. Hold, then straighten. Repeat 10 times.
  • Straight Leg Raises: Tighten your thigh, lift the whole leg 15cm off the bed. Hold 5 seconds, lower slowly.
  • Seated Knee Extensions: Sitting in a chair, straighten your knee fully. Hold 5 seconds, lower slowly.

Cost of Post-TKR Physiotherapy in Malaysia

Expect 15–25 physiotherapy sessions over 3–6 months. At private clinics, this typically costs RM2,000–RM5,000 total. Post-surgery rehabilitation may be partially covered by your medical insurance or SOCSO if the procedure was work-related.
